Resilience and high reliability are phrases that have become common in the last ten years. Organizations often ask, “What must we do to be resilient?” In fact, that’s probably the wrong question. The correct one is: “what kind of organization should we become?”

The most critical point at the heart of any resilient organization is a culture that is based on the acceptance of responsibility.
